Fístula aortoentérica primaria

Resumen

Las fístulas aortoentéricas son una causa infrecuente de hemorragia digestiva. Presentamos un paciente de 56 años de edad con una fístula aortoduodenal primaria que consultó por melena y masa pulsátil abdominal. Fue sometido a tres endoscopias digestivas, sin poder arribarse al diagnóstico. Una tomografía axial computada (TAC) y una arteriografía evidenciaron un aneurisma de aorta abdominal. En una laparotomía exploradora se identificó una fístula entre el aneurisma y la cuarta porción del duodeno. Se efectuó un baypass aortoaórtico y una reparación quirúrgica del duodeno. El paciente fue dado de alta al sexto día postoperatorio

Abstract

Primary aortoenteric fistula. Aortoenteric fistulas are a very unlikely cause of digestive bleeding. We present the case of a fifty six year old patient with a primary aortoduodenal fistula who consulted because of melena and a pulsating abdominal mass. He underwent three digestive endoscopies without arriving at any diagnosis. A computed axial tomography and an angiography showed an aneurism of the abdominal aorta. With an exploring laparotomy a fistula was identified between the aneurism and the fourth portion of the duodenum. An aortoaortic bypass was carried out followed by a surgical fix of the duodenum. The patient was released six days after the operation.
